Carla Lynn Hazelle Smith-Johnson was born on September 06, 1984 to Aniece Linda Chretien-Smith and Charles Joseph Smith in Chicago, Illinois. Carla departed this life on October 19, 2018 surrounded by close family and friends.

Carla grew up in Chicago, Illinois and visited her relatives in Louisiana often. Carla attended elementary school FXW Catholic School, graduated high school with honors from the Latin School of Chicago in June 2003, then went on to college. Carla attended Knox College in Galesburg, Illinois for 2 years, then attended Chicago State University where she received a Bachelor Arts Degree in Psychology with honors in May, 2012.

Carla Lynn married the love of her life, Jeffrey Johnson and of that union bore two sons Jayce Orion Johnson on June 20, 2007 and Jax Titan Johnson September 06, 2008. Carla was a wonderful, loving wife, daughter and mother. Carla was a stay-at-home mom where she home-schooled her son, Jayce, who is diagnosed with Autism. She patiently and lovingly dedicated her life to making sure Jayce and Jax excelled academically far above average grade levels. Carla was sweet, kind and compassionate to everyone she encountered. Carla was a very compassionate caregiver for her father who passed away one year ago. Carla always maintained a very close relationship with her mother and father, Charles and Aniece Linda, who loved their only child dearly. Carla also loved animals and had many pets and rescued animals with PAWS organization.

Carla Lynn Hazelle Smith-Johnson's legacy will live on through her husband, Jeffrey Johnson, her two sons, Jayce Orion Johnson and Jax Titan Johnson. Carla is lovingly survived by her mother, Aniece Linda Smith, her grandmother, Hazel G. Chretien and her in-laws, Drucella and Ronald Stingley and Ezzie Johnson; three sister-in-laws, Elizabeth Fefer, Toni Hairston, Ray Johnson. She is also survived by many aunts, uncles and cousins.

Carla was preceded in death by her father, Charles Joseph Smith, her maternal Grandfather, Leroy Chretien, her paternal grandparents, Ida Moody Smith and J.D. Smith.
